Healthcare Support is seeking a skilled LPN / LVN to provide home health services in Portland, Oregon. This is a 13-week contract opportunity with the possibility of extension.

Key Responsibilities:

--- Provide high-quality home health care services to patients in Portland, OR

--- Collaborate with healthcare teams to develop and implement patient care plans

--- Conduct thorough assessments and evaluations to identify patient needs

--- Develop and implement individualized care plans to meet patient goals

Requirements:

--- Active Professional licensure within the state of Oregon

--- 1 year of recent experience in home health care

--- BLS/ACLS Certification from the American Heart Association
