Health and Wellness Coordinator RoleAt The Ackerly at Timberland, our mission is to provide exceptional care to our residents, and we're seeking a skilled Health and Wellness Coordinator to help us achieve this goal. As a key member of our team, you will play a vital role in coordinating and delivering high-quality personal care services to our residents.
Key Responsibilities:

• Develop and implement effective care plans to meet the unique needs of each resident.
  
• Collaborate with direct care staff to ensure seamless delivery of care services.
  
• Manage staffing schedules and provide guidance on staffing needs to promote efficient care.
  
• Support the Health and Wellness Director/Manager as needed to ensure the smooth operation of our care services.
  Requirements:
  • Proven experience in care coordination or a related field.
    
  • Excellent communication and time management skills.
    
  • Geriatric knowledge and experience a plus.
    
  • Minimum of two years of supervisory experience preferred.
    
  • Associate's Degree in a health-related field recommended.
